While most on-campus surveys will probably be exempt from IRB review, everyone must submit the IRB form.

Take your form to one of the Research Coordinators below. They will determine if you need further IRB review. Once your survey has been reviewed and approved from one of these Research Coordinators (below), please confirm approval with the CIS Web Development Team (webdev@spu.edu).

Survey Name: What would you like to call your survey?
Survey Info: What should people know (i.e. instructions) about your survey before they begin?
Done URL: What URL should they be forwarded to when they are done? (Please include the 'http://' part of the URL)
Password: Does your survey require that only those who have your password can take it? If so, type the password here.
Make Survey Secure: Do you want your survey run in a secure environment (i.e. SSL)?
Contact: What kind of contact information should they have, if they have questions or problems with the survey?
Leave Message: What should people read when they are finished?
Survey Start Date: What date would you like people to be able to start taking your survey?
Survey End Date: What date should people no longer be able to take your survey? (Even if you don't know what your end date will be, please add a date far in the future.)

So you can add, modify or view results, please type in your email and a password for administrative priveleges.
