Application Checklist

This is a two-part application. This checklist will help you make sure you have provided all necessary information. Applications will not be considered if all elements are not completed. If you are having problems completing any of the requirements, email 37five@spu.edu.

  • Email Part 1:
    Email application in an attachment to 37five@spu.edu between April 4 and May 23.

    • Email your group picture in an attachment to 37five@spu.edu. (Preferably attached in the same email as your Part 1: Main Application)


    • Turn in all information on the organization that the application asks for. This includes name, address, website, contact name and email and EIN.


  • Turn in Part 2:
    Fulfillment page with signatures of all members to the Office of University Ministries (Second Floor SUB) no later than 5 p.m. on May 30. Turn this in as a hard copy.
