Parent/Guardian Statement Preference Form
If your student has completed a FERPA authorization giving you access to their account, you will automatically receive a paper copy of the monthly statement.
If your student has set you up as an Authorized Payer in the QuickPay system, you will receive an e-mail when new statements are available online at the e-mail address you have specified in your user preferences.

you are a parent or guardian currently receiving both paper and electronic statements, and you wish to have electronic only |
| * |
you are a parent or guardian who has previously discontinued paper statements, and wish to receive them again |
Requests will be effective for next month's statements.
