Parent/Guardian Statement Preference Form
Beginning March 2009, statements previously delivered via paper will be emailed as a PDF.
If your student has completed a FERPA authorization giving you access to their account, you will automatically receive a PDF via email to the email address provided by the student.
If your student has set you up as an Authorized Payer in the QuickPay system, you will receive an e-mail when new statements are available online at the e-mail address you have specified in your user preferences.

you are a parent or guardian who has both authorizations (PDF and online) and wish to discontinue receiving PDFs |

you are a parent or guardian who wants to get a paper statement instead of a PDF |

you are a parent or guardian who is currently getting paper statements, and would like PDF or online only instead |
Requests will be effective for next month's statements.
