This form is for letting us know about any changes in parking information. This includes new license plate information, new car, different parking lot preference, or any other information you might have that has changed. It can also be used to submit any questions about parking you may have. While you can request that a decal can be cancelled using this form, you will not be refunded until the decal is received by the Office of Safety and Security.
