Vision

The University offers a vision plan for all eligible, regular employees and their dependents. The vision plan uses the Vision Service Plan (VSP) network of providers. A detailed description of the program benefits, including limitations and exclusions, is found in the Vision Summary Highlights and theVision Certificate of Coverage both found here or by contacting the Office of Human Resources.

Identification and Filing Claims

VSP identifies employees and their covered dependents using the employee's SPU Identification number. There are no identification cards for this plan.  A higher level of benefits is paid when a provider is seen from within the VSP network. As you are required to pay the provider in full at the time of your appointment, you would need to submit a claim to VSP for partial reimbursement for any out-of-network or non-participating doctors.  Please contact VSP directly prior to utilizing the Claim Form for out-of-network or non-participating doctors.

Cost of Plan

The University pays the full premium for regular employees' coverage, regardless of FTE.  This is subject to change each year, based on renewal of program contracts.

The employee rate (as paid by the University) is noted below.

Employees who elect dependent coverage pay for the entire cost of their dependents' premiums by payroll deduction.

All vision insurance premiums deducted from employee earnings are taken on a pretax basis.
